I read your dissertation for your EdD and am absolutely fascinated with the topic, as well as your present article. I spent most of the afternoon trying to find out your contact data. The purpose of my contact was to seek your permission to use the survey instrument you authored for use in your dissertation.

My interest is in using the survey for a parallel study in several other states, perhaps with follow up interviews on a selective basis. Your thought process for your dissertation
is very much like my own. I concluded that I might have to formulate a survey instrument because of the lack of such info in the literature. Anyway, your dissertation is right on the money for my purposes.

I would greatly appreciate your assistance.
